Mr. Williams, a 79-year-old Medicare beneficiary, lives in an urban neighborhood and arrives to the nearby Academic Dental Center. His last dental visit was over two years ago. He was previously diagnosed with hypertension, type 2 diabetes, and depressive and anxiety disorders. The nurse practitioner and dentist team collaborate to promote a person-centered, integrated approach to care.
